Hockey4aCure Team Registration Form

Thank you for your interest in our 3 on 3 tournament to benefit the JDRF

This year 2002 – 2007 players may register.
Each age group is subject to registering at least 4 teams.

We are limited to 36 teams and expect to fill up quickly. If you would like to participate and do not have a team, we will attempt to assign you to a team.

The participation fee for each player is $80, all of which will go directly to the JDRF. Each participant will receive a jersey, a raffle ticket for an autographed NHL jersey (limited to participants only), and lots of food and fun!

Each team will play a minimum of 3 twenty-minute cross-ice games!
